Is Toniann Hart, FNP the Right Provider for You?

Choosing a primary care provider is a big decision. These FAQs are here to show how I practice: attentive, evidence-based, and truly patient-centered. My goal is to listen carefully, respond promptly, and care for you and your family the same way I expect my own family to be treated.

Credentials & Professional Background

1. What does MSN, APRN, FNP-BC, and FNP-C mean—how are you different from a physician or PA?

MSN means I have a Master of Science in Nursing, and APRN stands for Advanced Practice Registered Nurse. FNP-BC and FNP-C indicate that I am board-certified as a Family Nurse Practitioner by two national certifying bodies. I diagnose, treat, and manage many of the same conditions as a family physician, with a strong focus on education, prevention, and partnering with you in your ongoing care.

2. How many years have you been practicing as a board-certified Family Nurse Practitioner?

I have more than 13 years of experience in nursing, with one year spent practicing as a board-certified Family Nurse Practitioner. That time has allowed me to care for patients across the lifespan, from children to older adults, in a variety of clinical settings.

3. What inspired you to become a Family Nurse Practitioner and focus on whole-person care?

I became a Family Nurse Practitioner because I wanted to care for patients and families over time, not just for a single visit. I’ve seen how much better outcomes can be when we look at the whole person—their story, stressors, lifestyle, and goals—not just a diagnosis. My goal is to create space where patients feel heard, respected, and supported in making sustainable changes.

4. Do you have special training in any areas?

As a Family Nurse Practitioner, I’m trained to care for patients across the lifespan, including pediatrics, adult medicine, and older adults. My experience includes managing chronic conditions, women’s health needs, preventive care, and supporting patients with stress, anxiety, and mood concerns as they relate to their overall health.

Care Philosophy & Approach

5. What does “whole-person care” mean to you in everyday practice?

Whole-person care means we look beyond a single symptom or lab value. I take into account your physical health, emotional wellbeing, lifestyle, family context, and personal goals. Together, we build a plan that respects both the latest evidence and what is realistic for your daily life.

6. How do you make time to really listen to patients when visits are short?

I prioritize listening from the very beginning of each visit, asking focused questions and giving you room to share what matters most. I also use follow-up visits, patient education, and clear written plans to make sure you leave feeling heard and not rushed. When something is complex, we may schedule additional time so we can address it thoroughly.

7. How do you involve the whole family in creating a health plan?

Health decisions rarely affect just one person. When appropriate, I encourage family members or caregivers to be part of the conversation so everyone understands the plan and can support it at home. My goal is to help the entire household move toward healthier, more sustainable habits together.

8. What’s your approach when a patient’s symptoms don’t fit a textbook diagnosis?

When symptoms don’t fit neatly into a single diagnosis, I slow down and look at the broader picture. That may include reviewing your history in more depth, ordering targeted testing, and considering how stress, sleep, nutrition, or other factors could be contributing. We work step by step to narrow down root causes instead of simply masking symptoms.

Scope of Care Across the Lifespan

9. Can you be the primary provider for babies, kids, teens, adults, and seniors?

Yes. As a Family Nurse Practitioner, I’m trained in primary care for patients across the lifespan—from children to older adults. That allows me to care for multiple generations within the same family and better understand how health patterns and needs connect across your household.

10. What are the most common conditions you help families manage day-to-day?

I regularly help patients manage conditions such as high blood pressure, diabetes, thyroid concerns, asthma, respiratory infections, women’s health issues, and common pediatric illnesses. I also focus on prevention, lifestyle support, and early intervention to reduce long-term complications whenever possible.

Women’s Health, Mental Health & Chronic Conditions

11. Do you provide well-woman exams, birth control counseling, or menopause care?

Yes. I provide routine well-woman exams, screening and prevention services, and counseling about birth control options. I also support women navigating perimenopause and menopause, working with them to address symptoms and maintain long-term health based on current evidence and their personal preferences.

12. How do you help patients with anxiety, depression, or stress alongside physical symptoms?

I recognize that mental and physical health are closely connected. When anxiety, depression, or stress are part of the picture, we talk about how they’re affecting your body, daily routines, and relationships. I may recommend counseling, lifestyle strategies, medication when appropriate, and close follow-up so you feel supported, not judged.

Patient Experience & Communication

13. What do patients say about their experience in your care?

Patients often tell me they feel listened to, not rushed, and that I explain things in a way that finally makes sense. Many share that they appreciate having a provider who remembers their story and follows up on what we discussed at prior visits. My goal is for every patient to feel respected and cared for, not like a number.

14. How do you help patients feel less intimidated by medical jargon or bad past experiences?

I avoid jargon whenever possible and explain test results, diagnoses, and treatment options in plain language. I also acknowledge when someone has had a difficult experience with healthcare in the past and invite them to share what went wrong. Together, we rebuild trust by moving at a pace that feels safe and transparent.

15. What’s one change you’ve helped a patient make that transformed their health?

Without sharing identifying details, I’ve seen patients make powerful changes by taking small, consistent steps—such as improving sleep routines, adjusting nutrition, or sticking with a blood pressure plan. Often, the biggest transformation comes when patients feel heard and supported enough to believe that change is actually possible for them.

16. What makes your care feel personal instead of “assembly-line” medicine?

I work hard not to be another barrier in a process that already takes time. I review your chart before visits, respond to messages as promptly as I can, and place needed orders and referrals quickly so you are not left waiting on me. If a medication is too expensive or not covered, I do my best to find alternatives that fit your insurance and budget. When someone dislikes taking pills, we look together at whether other forms of the medicine are safe and appropriate. My goal is for you to feel that your safety, comfort, and needs are always at the center of the plan.

17. How quickly do you respond to patient messages, results, and follow-up needs?

I know waiting for answers can be stressful, so I do my best to respond as quickly as possible. I try to place referrals, lab orders, and prescription changes promptly so that I am not the hold-up in a process that is already complicated by insurance and scheduling. While there are things I can’t control, I take your time and concerns seriously and work hard to be attentive and responsive.

Trust, Fit & Long-Term Partnership

18. Why do families in Clarksville and beyond keep choosing you as their provider?

Families tell me they value having a provider who knows them, remembers their history, and truly cares about their long-term wellbeing. I aim to combine clinical thoroughness with a calm, approachable style so visits feel relational, not transactional.

19. How do you build trust with new patients who are nervous or skeptical?

I start by listening and validating their concerns, especially if they’ve felt dismissed in the past. I’m honest about what I know, what we need to learn more about, and the options we have. Trust grows over time as patients see that I follow through, remember what matters to them, and include them in every decision.

20. How do you stay current with the latest guidelines so families get evidence-based care?

I regularly review updated clinical guidelines, attend continuing education, and learn from reputable professional organizations. When recommendations change, I work with patients to update their care plans in a way that makes sense for their specific situation and values.

21. If I’m looking for a provider who treats me like family, how do I know you’re the right fit?

As a wife and mother, my family has walked through our own medical needs and tough seasons. I care for patients the way I want my own family to be treated—with honesty, respect, careful attention, and follow-through. If you want a provider who listens, explains, and partners with you on a realistic plan, there’s a good chance we’ll be a strong fit.
